Assumption of Risk

In agreeing to participate in the activities at Northeast Brazilian Jiu-Jitsu & MMA LLC, I affirm that my general health is good. I also affirm I am not affected by exercise and that I am capable of performing exercise of a moderate nature and/or performing the training and exercise required for martial arts. I am aware of the possibility of accidental or other physical injury during my exercise program.

In consideration with Northeast Brazilian Jiu-Jitsu & MMA LLC, I do hereby agree to assume all risks of such injury and will hold harmless from any and all liability, actions, claims and demands of every kind of nature whatsoever, which I now have or may arise in connections with any participation in activities arranged by Northeast Brazilian Jiu-Jitsu & MMA LLC, it’s employees and it’s staff. The terms herein shall serve as a release and assumption of risk for my heirs, executors and administrators for all members of my family, including minors.

I agree to wear appropriate workout/training clothes, uniform, shoes, and provide water during my workout/training sessions.

I grant permission to North East BJJ MMA, its representatives, employees, volunteers, and agents to take photographs, videos, recordings and/or audio recordings of me.

I have read this agreement and understand the exercise/training in which I will be engaged. I have agreed to the conditions above.
